Certificate of Death

We have death certificates for those who died in Royal Oak. Anyone may pick up a certified copy. You may come in person and pick them up the same day or send us a mail-in request.

For mail-requests, we need the following information: the name of the deceased, date of birth and date of death and a check made payable to the City of Royal Oak (Use the form below for your request).

Fees for the death certificates are as follows: $15.00 for the first copy and $5.00 for each additional copy.
